The Cardiac Rehabilitation Program puts patients on the path to a healthier lifestyle by promoting recovery and independence, while educating for prevention of cardiac disease. The program has been developed according to national standards established by the American Association of Cardiovascular and Pulmonary Rehabilitation. Through rehabilitation, patients can learn how to reduce many of the risk factors that contribute to heart disease such as inactivity, high blood pressure, high cholesterol, excess weight, smoking, diabetes and stress. such as: What are the goals of a Cardiac Rehabilitation Program? How does the Cardiac Rehabilitation Program work? Phase I (Inpatient) — While in the hospital, the Phase I staff will direct your early activity program and provide you with education about the recovery process and steps you can take to reduce your future risk of heart problems. Phase II (Outpatient) — Soon after you leave the hospital you can enter the outpatient Phase II program. Prior to entering the Phase II program, your risk factors for heart disease will be evaluated. We will develop a personal plan with you and your family, directed at strategies and education to lessen your risks. As part of the program, monitored exercise sessions are offered. Your exercise is designed to help you return to your daily activities, job, hobbies and independent lifestyles safely. Your program will be tailored to your personal needs, ability and interest. Phase III (Maintenance) — The Phase III program is an ongoing option for those that have completed the Phase II program. This program is designed to maintain the functional level and benefits achieved during the Phase II program. The Phase III program, which involves a small monthly membership fee, is an option for individuals who want to continue their exercise program and social interaction. How do you begin a Cardiac Rehabilitation Program? Cardiac Rehabilitation (outpatient) is covered by most insurance companies.
